Should primary cancer be treated in metastatic patients?

The metastatic clone cannot yet be identified in the primary tumor. Circulating tumor cells in localized disease are rare and could not earlier be used to predict metastases. Phase II data suggests that local treatment in metastasized prostate cancer may be beneficial, and retrospective data suggests that men with a prior local treatment had a better outcome in the metastasized stage. Stronger evidence from phase III trials is needed to support the use of local treatment in metastasized disease.
