Evaluation of pain in office based transperineal targeted prostate biopsy under local anesthesia using a modified anesthetic protocol

Introduction & Objectives

To evaluate pain during office based targeted transperineal prostate biopsy TTPB) under a modified local anesthetic protocol.

Materials & Methods

Between March 2019 and July 2019 we prospectively evaluated pain during TTPB applying a modified anesthetic protocol. 39 patients had office based TTPB. We used 40 ml of Lidocaine 1% diluted in 40 ml of saline solution, with 2 ml of Bicarbonate. 20 ml was used for subcutaneous anesthesia, 60 ml for deeper anesthesia distributed according to a specific protocol in relation with the biopsy planning. We used the Hand free Koelis system with trinity station. At the end of the procedure, the global pain was assessed through an analogic visual scale. Patients were evaluated 15 days after prostate biopsy to identify eventual complications.

Results

Average global pain was 1,3/10 (0-3). We reported no infection, significant bleeding or Acute urinary retention.

Conclusions

TTPB can be performed in outpatient clinic with an excellent tolerance using a modified anesthetic protocol.
